Consumer Information All athletic trainers must be licensed in Arizona and must meet the requirements of statute and rule.  Anyone receiving the services of an athletic trainer should contact the Board to determine if the athletic trainer is licensed and the license is in good standing. Persons wishing to file a complaint against an athletic trainer should file a complaint form with the Board. For assistance in filing a complaint and determining if the Board has jurisdiction you may want to contact the Board office at (602) 589-8352.  The board by statue accepts complaints, investigates, and takes any needed disciplinary action against athletic trainers for which it has jurisdiction.
